During this time of COVID-19, many people are thinking about their own end-of-life plans. This page includes a link to the church’s Life Crisis Form that will give Rev. Karen Bush some basic information to help you and your loved ones in the time of crisis. It also includes resources for you to use to make your end-of-life wishes known to loved ones as well as information regarding final arrangements.

This certainly is not a comprehensive list, but it will help you to start—or continue—to get your wishes down in writing and to have important conversations with loved ones.

If you have questions that are specific to your situation, you may wish to contact your physician or lawyer, as appropriate.

The Life Crisis Form is specific to our church. We encourage you to complete this form so that Reverend Karen Bush will have information in the event of an emergency. If you prefer to print out a blank copy of this form and fill it out, please feel free to do so and then return it to the Minister.

Below you will find many resources to plan for end-of-life care, including resources that focus on the current COVID pandemic. Many of these resources are from Compassion & Choices, a national group whose mission is to educate about and expand options regarding end-of-life care


Resources to Plan for Final Arrangements

The Funeral Consumers Alliance of Champaign County is a local affiliate of the national organization, Funeral Consumers Alliance. The websites below include information from this organization as well as information from other organizations regarding final arrangements such as finding a funeral home, burial decisions, body donation, and “green” burial.
