End-of-Life Planning Resources

During this time of COVID-19, many people are thinking about their own end-of-life plans. Some may be thinking about decisions they may need to make should they become sick, others may be realizing that they haven’t thought about–or communicated to loved ones–what types of interventions they might like to have (or not have) at the end of life. Two out of three U.S. adults have not completed an advance directive.

Many know they need to make plans, but haven’t and need a ‘nudge’. Consider this your ‘nudge’.

We have created a webpage that includes resources to help you think about and document end-of-life wishes. The church’s “Life Crisis Form” is also included, and we encourage you to complete it so that Rev. Caplow will have the information in the event of an emergency.

Let’s work together to encourage each other to have these important conversations and to complete the needed paperwork.
